Sajda Sahw (The prostration for forgetfulness)
The two sajdahs (prostrations) are called for the forgetfulness of the prayer is known as Sajda Sahw (The prostration for forgetfulness).

حَدَّثَنَا يَحْيَى بْنُ يَحْيَى، قَالَ قَرَأْتُ عَلَى مَالِكٍ عَنِ ابْنِ شِهَابٍ، عَنْ عَبْدِ الرَّحْمَنِ الأَعْرَجِ، عَنْ عَبْدِ اللَّهِ ابْنِ بُحَيْنَةَ، قَالَ صَلَّى لَنَا رَسُولُ اللَّهِ صلى الله عليه وسلم رَكْعَتَيْنِ مِنْ بَعْضِ الصَّلَوَاتِ ثُمَّ قَامَ فَلَمْ يَجْلِسْ فَقَامَ النَّاسُ مَعَهُ فَلَمَّا قَضَى صَلاَتَهُ وَنَظَرْنَا تَسْلِيمَهُ كَبَّرَ فَسَجَدَ سَجْدَتَيْنِ وَهُوَ جَالِسٌ قَبْلَ التَّسْلِيمِ ثُمَّ سَلَّمَ .
'Abdullah b. Buhaina reported:
The Messenger of Allah (ﷺ) led us two rak'ahs of prayer in one of the (obligatory) prayers and then got up and did not sit. and the people stood up along with him. When he finished the prayer and we expected him to pronounce salutation. he said:" Allah is Most Great" while sitting and made two prostrations before salutation and then pronounced (the, final) salutation.

حَدَّثَنَا أَبُو الْيَمَانِ، قَالَ أَخْبَرَنَا شُعَيْبٌ، عَنِ الزُّهْرِيِّ، قَالَ حَدَّثَنِي عَبْدُ الرَّحْمَنِ بْنُ هُرْمُزَ، مَوْلَى بَنِي عَبْدِ الْمُطَّلِبِ ـ وَقَالَ مَرَّةً مَوْلَى رَبِيعَةَ بْنِ الْحَارِثِ ـ أَنَّ عَبْدَ اللَّهِ ابْنَ بُحَيْنَةَ ـ وَهْوَ مِنْ أَزْدِ شَنُوءَةَ وَهْوَ حَلِيفٌ لِبَنِي عَبْدِ مَنَافٍ، وَكَانَ مِنْ أَصْحَابِ النَّبِيِّ صلى الله عليه وسلم. أَنَّ النَّبِيَّ صلى الله عليه وسلم صَلَّى بِهِمُ الظُّهْرَ فَقَامَ فِي الرَّكْعَتَيْنِ الأُولَيَيْنِ لَمْ يَجْلِسْ، فَقَامَ النَّاسُ مَعَهُ حَتَّى إِذَا قَضَى الصَّلاَةَ، وَانْتَظَرَ النَّاسُ تَسْلِيمَهُ، كَبَّرَ وَهْوَ جَالِسٌ، فَسَجَدَ سَجْدَتَيْنِ قَبْلَ أَنْ يُسَلِّمَ ثُمَّ سَلَّمَ.
Narrated `Abdullah bin Buhaina:
(he was from the tribe of Uzd Shanu'a [??] and was the ally of the tribe of `Abdul-Manaf and was one of the companions of the Prophet): Once the Prophet (ﷺ) led us in the Zuhr prayer and stood up after the second rak`a and did not sit down. The people stood up with him. When the prayer was about to end and the people were waiting for him to say the Taslim, he said Takbir while sitting and prostrated twice before saying the Taslim and then he said the Taslim."

If rak’ats got increased….
حَدَّثَنَا أَبُو الْوَلِيدِ، حَدَّثَنَا شُعْبَةُ، عَنِ الْحَكَمِ، عَنْ إِبْرَاهِيمَ، عَنْ عَلْقَمَةَ، عَنْ عَبْدِ اللَّهِ ـ رضى الله عنه ـ أَنَّ رَسُولَ اللَّهِ صلى الله عليه وسلم صَلَّى الظُّهْرَ خَمْسًا فَقِيلَ لَهُ أَزِيدَ فِي الصَّلاَةِ فَقَالَ " وَمَا ذَاكَ ". قَالَ صَلَّيْتَ خَمْسًا. فَسَجَدَ سَجْدَتَيْنِ بَعْدَ مَا سَلَّمَ.
Narrated' `Abdullah:
Once Allah's Messenger (ﷺ) offered five rak`at in the Zuhr prayer, and somebody asked him whether there was some increase in the prayer. Allah's Messenger (ﷺ) said, "What is that?" He said, "You have offered five rak`at." So Allah's Messenger (ﷺ) performed two prostrations of Sahu after Taslim.

If rak’ats got reduced...
حَدَّثَنَا إِسْحَاقُ، قَالَ حَدَّثَنَا ابْنُ شُمَيْلٍ، أَخْبَرَنَا ابْنُ عَوْنٍ، عَنِ ابْنِ سِيرِينَ، عَنْ أَبِي هُرَيْرَةَ، قَالَ صَلَّى بِنَا رَسُولُ اللَّهِ صلى الله عليه وسلم إِحْدَى صَلاَتَىِ الْعَشِيِّ ـ قَالَ ابْنُ سِيرِينَ سَمَّاهَا أَبُو هُرَيْرَةَ وَلَكِنْ نَسِيتُ أَنَا ـ قَالَ فَصَلَّى بِنَا رَكْعَتَيْنِ ثُمَّ سَلَّمَ، فَقَامَ إِلَى خَشَبَةٍ مَعْرُوضَةٍ فِي الْمَسْجِدِ فَاتَّكَأَ عَلَيْهَا، كَأَنَّهُ غَضْبَانُ، وَوَضَعَ يَدَهُ الْيُمْنَى عَلَى الْيُسْرَى، وَشَبَّكَ بَيْنَ أَصَابِعِهِ، وَوَضَعَ خَدَّهُ الأَيْمَنَ عَلَى ظَهْرِ كَفِّهِ الْيُسْرَى، وَخَرَجَتِ السَّرَعَانُ مِنْ أَبْوَابِ الْمَسْجِدِ فَقَالُوا قَصُرَتِ الصَّلاَةُ. وَفِي الْقَوْمِ أَبُو بَكْرٍ وَعُمَرُ، فَهَابَا أَنْ يُكَلِّمَاهُ، وَفِي الْقَوْمِ رَجُلٌ فِي يَدَيْهِ طُولٌ يُقَالُ لَهُ ذُو الْيَدَيْنِ قَالَ يَا رَسُولَ اللَّهِ، أَنَسِيتَ أَمْ قَصُرَتِ الصَّلاَةُ قَالَ " لَمْ أَنْسَ، وَلَمْ تُقْصَرْ ". فَقَالَ " أَكَمَا يَقُولُ ذُو الْيَدَيْنِ ". فَقَالُوا نَعَمْ. فَتَقَدَّمَ فَصَلَّى مَا تَرَكَ، ثُمَّ سَلَّمَ، ثُمَّ كَبَّرَ وَسَجَدَ مِثْلَ سُجُودِهِ أَوْ أَطْوَلَ، ثُمَّ رَفَعَ رَأْسَهُ وَكَبَّرَ، ثُمَّ كَبَّرَ وَسَجَدَ مِثْلَ سُجُودِهِ أَوْ أَطْوَلَ، ثُمَّ رَفَعَ رَأْسَهُ وَكَبَّرَ. فَرُبَّمَا سَأَلُوهُ ثُمَّ سَلَّمَ فَيَقُولُ نُبِّئْتُ أَنَّ عِمْرَانَ بْنَ حُصَيْنٍ قَالَ ثُمَّ سَلَّمَ.
Narrates Ibn Seereen:
Abu Huraira said, "Allah's Messenger (ﷺ) led us in one of the two `Isha' prayers (Abu Huraira named that prayer but I forgot it)." Abu Huraira added, "He prayed two rak`at and then finished the prayer with Taslim. He stood up near a piece of wood Lying across the mosque and leaned on it in such a way as if he was angry. Then he put his right hand over the left and clasped his hands by interlacing his fingers and then put his J right cheek on the back of his left hand. The people who were in haste left the mosque through its gates. They wondered whether the prayer was reduced. And amongst them were Abu Bakr and `Umar but they hesitated to ask the Prophet. A long-handed man called Dhul- Yadain asked the Prophet, 'O Allah's Messenger (ﷺ)! Have you forgotten or has the prayer been reduced?' The Prophet (ﷺ) replied, 'I have neither forgotten nor has the prayer been reduced' The Prophet (ﷺ) added, 'Is what Dhul Yadain has said true?' They (the people) said, 'Yes, it is true.' The Prophet (ﷺ) stood up again and led the prayer, completing the remaining prayer, forgotten by him, and performed Taslim, and then said, 'Allahu Akbar.' And then he did a prostration as he used to prostrate or longer than that. He then raised his head saying, 'Allahu Akbar; he then again said, 'Allahu Akbar', and prostrated as he used to prostrate or longer than that. Then he raised his head and said, 'Allahu Akbar.' " (The subnarrator added, "I think that they asked (Ibn Seereen) whether the Prophet (ﷺ) completed the prayer with Taslim. He replied, "I heard that `Imran bin Husain had said, 'Then he (the Prophet) did Taslim.")

If there is a doubt in the rak'at’s count ...
Was it three rak'ats that was completed in prayer? Or was it four rak’ats in the prayer. In such an event of suspicion, consider the lesser number among them. That is to calculate the remaining, with three rakat as being considered to be offered and the remaining one rak’at more to be offered in the prayer. And also along with this, two sajdahs (prostration) should be done.
وَحَدَّثَنِي مُحَمَّدُ بْنُ أَحْمَدَ بْنِ أَبِي خَلَفٍ، حَدَّثَنَا مُوسَى بْنُ دَاوُدَ، حَدَّثَنَا سُلَيْمَانُ بْنُ بِلاَلٍ، عَنْ زَيْدِ بْنِ أَسْلَمَ، عَنْ عَطَاءِ بْنِ يَسَارٍ، عَنْ أَبِي سَعِيدٍ الْخُدْرِيِّ، قَالَ قَالَ رَسُولُ اللَّهِ صلى الله عليه وسلم " إِذَا شَكَّ أَحَدُكُمْ فِي صَلاَتِهِ فَلَمْ يَدْرِ كَمْ صَلَّى ثَلاَثًا أَمْ أَرْبَعًا فَلْيَطْرَحِ الشَّكَّ وَلْيَبْنِ عَلَى مَا اسْتَيْقَنَ ثُمَّ يَسْجُدُ سَجْدَتَيْنِ قَبْلَ أَنْ يُسَلِّمَ فَإِنْ كَانَ صَلَّى خَمْسًا شَفَعْنَ لَهُ صَلاَتَهُ وَإِنْ كَانَ صَلَّى إِتْمَامًا لأَرْبَعٍ كَانَتَا تَرْغِيمًا لِلشَّيْطَانِ " .
Abu Sa'id al-Khudri reported:
The Messenger of Allah (ﷺ) said: When any one of you is in doubt about his prayer and he does Dot know how much he has prayed, three or four (rak'ahs). he should cast aside his doubt and base his prayer on what he is sure of. then perform two prostrations before giving salutations. If he has prayed five rak'ahs, they will make his prayer an even number for him, and if he has prayed exactly four, they will be humiliation for the devil.

حَدَّثَنَا قُتَيْبَةُ، حَدَّثَنَا اللَّيْثُ، عَنِ ابْنِ شِهَابٍ، عَنْ أَبِي سَلَمَةَ، عَنْ أَبِي هُرَيْرَةَ، قَالَ قَالَ رَسُولُ اللَّهِ صلى الله عليه وسلم " إِنَّ الشَّيْطَانَ يَأْتِي أَحَدَكُمْ فِي صَلاَتِهِ فَيَلْبِسُ عَلَيْهِ حَتَّى لاَ يَدْرِي كَمْ صَلَّى فَإِذَا وَجَدَ ذَلِكَ أَحَدُكُمْ فَلْيَسْجُدْ سَجْدَتَيْنِ وَهُوَ جَالِسٌ " . قَالَ أَبُو عِيسَى هَذَا حَدِيثٌ حَسَنٌ صَحِيحٌ .
Abu Hurairah narrated that:
Allah's Messenger (S) said: "Indeed the Shaitan comes to one of you in his Salat confusing him until he does now know how much he has prayed. When one of you experiences that then let him perform two prostrations while sitting."

حَدَّثَنَا عُثْمَانُ، قَالَ حَدَّثَنَا جَرِيرٌ، عَنْ مَنْصُورٍ، عَنْ إِبْرَاهِيمَ، عَنْ عَلْقَمَةَ، قَالَ قَالَ عَبْدُ اللَّهِ صَلَّى النَّبِيُّ صلى الله عليه وسلم ـ قَالَ إِبْرَاهِيمُ لاَ أَدْرِي زَادَ أَوْ نَقَصَ ـ فَلَمَّا سَلَّمَ قِيلَ لَهُ يَا رَسُولَ اللَّهِ، أَحَدَثَ فِي الصَّلاَةِ شَىْءٌ قَالَ " وَمَا ذَاكَ ". قَالُوا صَلَّيْتَ كَذَا وَكَذَا. فَثَنَى رِجْلَيْهِ وَاسْتَقْبَلَ الْقِبْلَةَ، وَسَجَدَ سَجْدَتَيْنِ ثُمَّ سَلَّمَ، فَلَمَّا أَقْبَلَ عَلَيْنَا بِوَجْهِهِ قَالَ " إِنَّهُ لَوْ حَدَثَ فِي الصَّلاَةِ شَىْءٌ لَنَبَّأْتُكُمْ بِهِ، وَلَكِنْ إِنَّمَا أَنَا بَشَرٌ مِثْلُكُمْ، أَنْسَى كَمَا تَنْسَوْنَ، فَإِذَا نَسِيتُ فَذَكِّرُونِي، وَإِذَا شَكَّ أَحَدُكُمْ فِي صَلاَتِهِ فَلْيَتَحَرَّى الصَّوَابَ، فَلْيُتِمَّ عَلَيْهِ ثُمَّ يُسَلِّمْ، ثُمَّ يَسْجُدْ سَجْدَتَيْنِ ".
Narrated `Abdullah:
The Prophet (ﷺ) prayed (and the sub-narrator Ibrahim said, "I do not know whether he prayed more or less than usual"), and when he had finished the prayers he was asked, "O Allah's Messenger (ﷺ)! Has there been any change in the prayers?" He said, "What is it?' The people said, "You have prayed so much and so much." So the Prophet (ﷺ) bent his legs, faced the Qibla and performed two prostration's (of Sahu) and finished his prayers with Taslim (by turning his face to right and left saying: 'As-Salamu `Alaikum- Warahmat-ullah'). When he turned his face to us he said, "If there had been anything changed in the prayer, surely I would have informed you but I am a human being like you and liable to forget like you. So if I forget remind me and if anyone of you is doubtful about his prayer, he should follow what he thinks to be correct and complete his prayer accordingly and finish it and do two prostrations (of Sahu).

If one suspects that three rak'ahs or four rak'ats have been worshiped in four rak'ahs, he must take three rak'aths and complete the fourth rak'at. Before giving salaam at the end of the fourth rakat "Allahu Akbar," saying that the need to prostrate two times. Or two Sajdas can be done after Salaam is given. There is no evidence that the Prophet (peace be upon him) had taught him to do so in order to recite in the Sajdah for forgetfullness. So always read sajdah Dua to be recited.
